In the many steps involved it is inevitable that errors will find
their way into registration listings. We are, however, aiming for a
high degree of accuracy in the registration lists that will be published
after the convention in the Proceedings.

Would you therefore please use this form to report any correction
in your personal listing in the front of this book? (Please do not re-
port a supposed error in someone else’s listing; ask him to do that.
Sometimes “errors” turn out not to be errors after all.)

When an altogether new name is to be inserted, this change must
be supported by documentation in the Secretary’s files.

In submitting this sheet, please detach the entire page instead of
merely the bottom half.
